Seniors 65 year of age and older, if you are having trouble paying for prescription drugs, you may qualify for extra help from Medicare.
Eligibility criteria are:
- Age 65 or greater
- Income at, or below, 125% of the Federal Poverty Guidelines.

To apply for service bring:
- Current Michigan Driver’s License or State Picture ID for all adults in the household (18 years of age and older)
- Social Security cards for everyone in the household
- Proof of income (for everyone in the home)
- Must be proof for the last 90 days
- Pension statement
- Current Social Security statement
- Bank Statements ARE NOT accepted
- If you, or anyone in your household, receives food stamps (FAP) benefit, documentation MUST BE provided
- Must be proof for the last 90 days
